By fostering this sense of community, we can help reduce healthcare costs by shifting the focus from hospital-based care to nurturing community-based support systems where people feel more at home.
This model particularly benefits at-risk populations, including those with life-limiting illnesses, the elderly, individuals with disabilities, and those who may feel isolated and vulnerable, along with their dedicated caregivers. These groups thrive when they receive holistic care that truly addresses their physical, mental, and social needs, prioritizes their quality of life, acknowledges the reality of mortality, and ensures that caregivers are also supported in their vital roles. OUR MISSION


Our mission is to compassionately shift community attitudes toward life-limiting illnesses and palliative care over the next 18 months. We believe that through community involvement and education, we can foster understanding and compassion.
By doing this, we hope to alleviate the fear and uncertainty that often come with these challenges, empowering community members, families, and friends to support one another in profound and meaningful ways.
We recognize that staying informed is crucial, and we are committed to weaving education and information throughout our efforts. By providing the knowledge needed, we can help prevent crises and address the unknown together.

To champion a social model of end-of-life care, focus on community-based approaches, interdisciplinary teamwork, and empowering individuals and families to make informed decisions about their care and support needs. Focus on building a community's ability to address end-of-life needs through education, training, and resource mobilization. Recognize that factors like socioeconomic status, access to resources, and social isolation can impact end-of-life experiences.
